
Below are summaries of recent decisions of the Ontario Physicians and Surgeons Discipline Tribunal (OPSDT).

Dr. Richard Nahas – CPSO #72330
Specialty: Family Medicine
On February 3, 2026, the Ontario Physicians and Surgeons Discipline Tribunal found that Dr. Nahas committed the following acts of professional misconduct:
- failing to maintain the standard of practice of the profession; and
- engaging in an act or omission relevant to the practice of medicine that, having regard to all the circumstances, would reasonably be regarded by registrants as disgraceful, dishonourable or unprofessional.
The above findings arise from two separate matters that were heard together by the Tribunal. The matters were referred on August 15, 2024, and April 10, 2025, respectively.
On the same date, the Tribunal ordered that:
- the registrant attend before the panel to be reprimanded;
- the Registrar suspend the registrant’s certificate of registration for 4 months commencing February 4, 2026 at 12:01 a.m.;
- the Registrar place terms, conditions and limitations on the registrant’s certificate of registration; and
- the registrant pay the College costs in the amount of $6,000 by March 3, 2026.

Dr. Ashwin Rajan Maharaj – CPSO #67100

On January 28, 2026, the Ontario Physicians and Surgeons Discipline Tribunal found that Dr. Maharaj committed the following acts of professional misconduct:
- failing to maintain the standard of practice of the profession;
- engaging in an act or omission relevant to the practice of medicine that, having regard to all the circumstances, would reasonably be regarded by registrants as disgraceful, dishonourable or unprofessional;
- contravening a term, condition or limitation on the registrant’s certificate of registration; and
- contravening the Act, the Regulated Health Professions Act, 1991 or the regulations under either of those Acts.
The Tribunal further found the registrant incompetent.
On the same date, the Tribunal ordered that:
- the registrant attend before the panel to be reprimanded;
- the Registrar revoke the registrant’s certificate of registration effective immediately; and
- the registrant pay the College costs in the amount of $6,000 by February 27, 2026.

Dr. Jai Jayaraman – CPSO #78814

On February 18, 2026, the Ontario Physicians and Surgeons Discipline Tribunal found that Dr. Jayaraman committed the following acts of professional misconduct:
- engaging in an act or omission relevant to the practice of medicine that, having regard to all the circumstances, would reasonably be regarded by registrants as disgraceful, dishonourable or unprofessional; and
- contravening a term, condition or limitation on the registrant’s certificate of registration.
On the same date, the Tribunal ordered that:
- the registrant attend before the panel to be reprimanded;
- the Registrar suspend the registrant’s certificate of registration for 4 months commencing February 19, 2026 at 12:01 a.m.;
- the Registrar place terms, conditions and limitations on the registrant’s certificate of registration; and
- the registrant pay the College costs in the amount of $6,000 by March 20, 2026.

Dr. Zaira Azher – CPSO #97414
Specialty: Family Medicine
On February 24, 2026, the Ontario Physicians and Surgeons Discipline Tribunal found that Dr. Azher committed the following acts of professional misconduct:
- engaging in an act or omission relevant to the practice of medicine that, having regard to all the circumstances, would reasonably be regarded by registrants as disgraceful, dishonourable or unprofessional; and
- the governing body of another health profession in Ontario, or the governing body of a health profession in a jurisdiction other than Ontario, has found that the member committed an act of professional misconduct that would, in the opinion of the panel, be an act of professional misconduct under this section or an act of professional misconduct as defined in the regulations.
On the same date, the Tribunal ordered that:
- the registrant attend before the panel to be reprimanded;
- the Registrar suspend the registrant’s certificate of registration for 2 months commencing February 25, 2026 at 12:01 a.m.; and
- the registrant pay the College costs in the amount of $6,000 by March 24, 2026.

Dr. Stefan Joseph Konasiewicz – CPSO #60999

On February 9, 2026, the Ontario Physicians and Surgeons Discipline Tribunal found that Dr. Konasiewicz committed the following acts of professional misconduct:
- failing to maintain the standard of practice of the profession;
- contravening a term, condition or limitation on the registrant’s certificate of registration; and
- engaging in an act or omission relevant to the practice of medicine that, having regard to all the circumstances, would reasonably be regarded by registrants as disgraceful, dishonourable or unprofessional.
On the same date, the Tribunal ordered that:
- the registrant attend before the panel to be reprimanded;
- the Registrar suspend the registrant’s certificate of registration for 6 months commencing February 10, 2026 at 12:01 a.m.;
- the Registrar place terms, conditions and limitations on the registrant’s certificate of registration; and
- the registrant pay the College costs in the amount of $6,000 by March 9, 2026.

Dr. Sam Naghibi – CPSO #94848
Specialty: Family Medicine
On March 4, 2026, the Ontario Physicians and Surgeons Discipline Tribunal found that Dr. Naghibi committed the following acts of professional misconduct:
- being found guilty of an offence relevant to the registrant’s suitability to practice;
- sexually abusing a patient;
- engaging in an act or omission relevant to the practice of medicine that, having regard to all the circumstances, would reasonably be regarded by registrants as disgraceful, dishonourable or unprofessional; and
- engaging in conduct unbecoming a physician.
On the same date, the Tribunal ordered that:
- the registrant attend before the panel to be reprimanded;
- the Registrar revoke the registrant’s certificate of registration effective immediately; and
- the registrant pay the College costs in the amount of $6,000 by April 4, 2026.

Dr. Aaron Francis Jolly – CPSO #110954

On November 21, 2025, the Ontario Physicians and Surgeons Discipline Tribunal found that Dr. Jolly committed the following acts of professional misconduct:
- failing to maintain the standard of practice of the profession; and
- engaging in an act or omission relevant to the practice of medicine that, having regard to all the circumstances, would reasonably be regarded by registrants as disgraceful, dishonourable or unprofessional.
On January 22, 2026, the Tribunal ordered that:
- the registrant attend before the panel to be reprimanded;
- the Registrar suspend the registrant’s certificate of registration for 5 months commencing January 23, 2026 at 12:01 a.m.;
- the Registrar place terms, conditions and limitations on the registrant’s certificate of registration; and
- the registrant pay the College costs in the amount of $30,000 by March 23, 2026.

On March 23, 2026, the Ontario Physicians and Surgeons Discipline Tribunal found that Dr. Iracleous committed the following acts of professional misconduct:
- failing to respond appropriately or within a reasonable time to a written inquiry from the College; and
- engaging in an act or omission relevant to the practice of medicine that, having regard to all the circumstances, would reasonably be regarded by registrants as disgraceful, dishonourable or unprofessional.
On the same date, the Tribunal ordered that:
- the registrant attend before the panel to be reprimanded;
- the Registrar revoke the registrant’s certificate of registration effective immediately; and
- the registrant pay the College costs in the amount of $6,000 by April 22, 2026.

- the Registrar place terms, conditions and limitations on the registrant’s certificate of registration; and
